Join the Practice
You are eligible to register with our Oak Vale Medical Centre if your home address is within the practice area.
Note: If you are registered with us, and you move outside of the practice area, you may be asked to register with another GP practice.
How to Register
You need to come down to the surgery and one of our care co-ordinators will ask you to fill out and sign a registration GMS1 form with your personal details. They will also ask you to complete a patient questionnaire which comprises of some basic lifestyle questions i.e. alcohol, smoking status and family history.
For purposes of identification you will need to bring photo ID e.g. passport; proof of address e.g. utility bill (gas, electricity or water) or bank statement, dated within the last 3 months.
Once you have registered your records will be sent to us from your previous practice.

New Patient Medical
We offer all our new patients a medical check with one of our Health Care Assistants (HCA). This will consist of a short medical examination e.g height, weight, blood pressure, urine testing, medication review and the HCA takes the opportunity to ask about previous medical history.
